If the EHR is not accessible due to a technology outage or if you are unable to create a treatment summary for any other reason, here is what you can do.

1. Please request that the patient signs a paper medical consent form. You may have these printed in your office in case you need to resort to this work around:

2. Have the patient fill out a medical intake/questionnaire if necessary:

3.  Capture before and after photos using the iPad camera.

4. Record all necessary information in the Manual Treatment Summary Form

Once the EHR service is reestablished, you may re-construct the treatment summary adding it to the patient’s profile along with photos of the printed signed consent and the rest of the documentation that was gathered manually during the appointment.
